Passports |
In order to renew your U.S. passport you will have to complete a passport application form. You should use form DSP-82 (click here) if you were last issued a passport (having 10 years validity) within the last 15 years and you can present that passport to us. You must use form DSP-11 (click here) for children (under 16), if your passport was lost, stolen or severely damaged, or if your last passport was issued more than 15 years ago and/or was valid for less than 10 years. If your more recent passport has been lost or stolen, you will also need to complete form DS-64 (click here). In addition to the completed application, you must present proof of your U.S. citizenship (your old passport, if you have it), and two 2 x 2 inch photographs of yourself (color with a clear, light-colored background) taken within the last six months. The normal "Passport" photos taken in Zambia are not large enough, so please ask for photos that are 2 x 2 inches. These photos should give a full face view in which the face takes up at least half the vertical space of the photo. Do not sign the photograph. Click here to see current passport fees. Please note that the US Embassy Lusaka only accepts payment in cash – either entirely in US dollars or the equivalent in Zambian kwacha. New law requires the appearance of and signature by both parents for children 16 years of age and under. Alternatively, one parent can appear and sign if the other parent has executed the consent form DS-3053 (click here). The appearing parent should bring both parents' passports.
